This notice describes the privacy practices of Chiroport LLC and the doctor of chiropractic and staff who provide care at our office at 919 Maryland Ave E, Suite A, Saint Paul, MN 55106.

- For treatment. To provide and coordinate your chiropractic care, including referrals to other providers.
- For payment. To bill and collect payment from your insurer (including MN no-fault), an attorney lien holder, or you directly.
- For health-care operations. Internal activities such as quality improvement, training, and compliance.
- As required or permitted by law. Including reporting required under Minnesota and federal law (e.g., suspected abuse, public-health reporting, court orders).
- With your written authorization. For any use or disclosure not described above.
Your rights
- The right to see and copy your record.
- The right to request a correction if you believe information is wrong.
- The right to a list of certain disclosures we have made.
- The right to request restrictions on how we use or share your information.
- The right to request confidential communications (e.g., contact you only at a certain phone number).
- The right to a paper copy of this notice on request.
- The right to file a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. We will not retaliate.

Marketing and the website
We do not use a Meta Pixel or other client-side advertising tracker on this website. We do not sell or share your protected health information with advertisers. Conversion measurement, when used, is done with server-side methods that do not transmit identifying health information to ad platforms.
