What this page is for
This page lets you request a callback from Chiroport to discuss chiropractic care after a car accident or other injury. Submitting the callback form does not create a doctor-patient relationship, schedule an appointment, or guarantee a specific treatment outcome.

SMS messaging program
The SMS program is optional and operated by Chiroport LLC. To enroll you must affirmatively check the SMS opt-in box on our callback form and provide your mobile phone number. Submitting the callback form by itself does not enroll you in SMS — you can request a callback without opting in to texts, and we will only call you. If you check the SMS opt-in box, you consent to receive SMS messages and calls from Chiroport about your inquiry, appointment, and care.
- Program description: appointment scheduling, confirmations, reminders, missed-call recovery, and post-visit follow-up.
- Message frequency: varies, typically 1–10 messages per month.
- Cost: Chiroport does not charge for SMS messages. Message and data rates may apply from your wireless carrier.
- Opt-out: Reply STOP to any message to unsubscribe. Reply START to opt back in.
- Help: Reply HELP for help, or call (651) 202-3178.
- Supported carriers: AT&T, T-Mobile, Verizon, Sprint, Boost Mobile, U.S. Cellular, MetroPCS, Google Fi, Cricket, and other major U.S. wireless carriers. Carriers are not liable for delayed or undelivered messages.
- Privacy: See our Privacy Notice. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
Acceptable use
You agree to provide accurate information on the callback form, to use this page only for its intended purpose (requesting chiropractic care), and not to submit information about another person without their permission.
Limitation of liability
Chiroport LLC provides this page "as is" without warranties of any kind. To the maximum extent permitted by law, Chiroport LLC is not liable for indirect, incidental, or consequential damages arising from your use of this page or the SMS messaging program. Nothing in these terms limits any rights you have under HIPAA or other applicable patient-protection laws.
Changes
We may update these terms from time to time. The "Last updated" date at the top of this page reflects the most recent change. Continued use of the page after a change constitutes acceptance of the updated terms.
Governing law
These terms are governed by the laws of the State of Minnesota.
